Procurement Card
Please visit the Procurement Card Program training Web site for on-line training. Potential cardholders and approvers must pass the Knowledge Assessment before submitting the application forms. If you prefer to have a form sent to you, contact the Procurement Card Administrator at 443-997-5650 or by email at pcard@jhu.edu.
Independent Contractor
- Independent Contractor Certification - For Services of $5,000 or more
 The department has primary responsibility for accurately completing part I of the Independent Contractor Certification form and making sure that part II is completed by the individual providing the service. Send completed form to the Tax Office - Short Form Independent Contractor Agreement - For Services less than $5,000
Should be used when a project is for a short duration, the services are less than $5,000 and payment is made in a single lump-sum. Send completed form to Accounts Payable - Independent Contractor Agreement - For Services of $5,000 or more
 Should be used when a project is for a short duration, the services are $5,000 or greater and less than $50,000 and when no intellectual property, computer software or web pages are expected to be produced. - Master Consulting Services Agreement - For Services of $5,000 or more
 Should be used when the cost of the project exceeds $50,000 or when intellectual property, other than computer software and web pages, is produced as a result of hire or for projects of longer duration.
- ICTSA - Independent Computer Technology Services Agreement - For Services of $5,000 or more
 Should be used when computer software or web pages are expected to be produced. Send completed form to Supply Chain
- International Independent Contractor Agreement - For International Services of $5,000 or more
Should be used when a project is for a short duration, the services are $5,000 or greater and less than $50,000 and when no intellectual property, computer software or web pages are expected to be produced. - International Independent Contractor Certification - For Services of $5,000 or more
 The department has primary responsibility for accurately completing part I of the International Independent Contractor Certification form and making sure that part II is completed by the individual providing the service. The information on this form is used to determine whether the services and individual comply with IRS independent contractor guidelines and laws of the country where the individual will provide the services on the University’s behalf.
